It is a holistic and multi-disciplinary approach based on working with individuals to support/promote their own wellbeing. Put simply, the aim is to help people improve their health and overall quality of life by supporting them to make more independent, pro-active choices, taking into account of all determinants of wellbeing in context of any existing health and social care support.
